At Hope Health & Care Services, we provide safe, supported and community style group homes for people with disability across New South Wales (NSW), Queensland (QLD) and Victoria (VIC). Our homes offer a comfortable, inclusive and welcoming living environment with 24/7 support, helping every resident enjoy independence, stability and a genuine sense of belonging. We believe every person with disability deserves a place they can truly call home – a place where they are respected, supported and empowered to live life on their own terms.

What Is a Disability Group Home?
A Disability Group Home is a shared residential arrangement where a small number of people with disability live together in a community setting, with on site support staff available around the clock. Group homes provide a structured, safe and socially connected living environment for participants who require regular support but want to maintain a level of independence and community participation.
At Hope Health & Care Services, our group homes are not just places to live – they are genuine homes. We focus on creating warm, personalised environments where every resident feels valued, included and supported to achieve their individual goals and live as independently as possible.
Group homes are funded under the NDIS through Supported Independent Living (SIL) funding, which covers the cost of the support and staffing within the home. In some cases, participants may also be eligible for Specialist Disability Accommodation (SDA) funding to cover the cost of the housing itself.
Who Our Group Homes Support
Our group homes are suited for NDIS participants who:

What We Provide: Services & Support
Living at an HH&CS group home offers more than accommodation, it is about living a fuller life. Our support includes:
- 24/7 on site support staff for assistance with everyday living, personal care, mobility, medication, and health needs
- Assistance with daily living tasks including personal care, meals, cleaning, household management, transport, and daily routines
- Community access and social participation including support to attend appointments, social events, community programs, educational or employment activities
- Support for complex needs for residents with higher levels of disability or additional support requirements such as mobility aids, health monitoring, behaviour support, and more
- Person centred living we support residents to make choices about their daily life, routines, social interactions, and community involvement

Our group homes provide 24/7 support including personal care, daily living assistance, meal preparation, household management, community participation, life skills development, health and wellbeing support and transport.
