At Hope Health & Care Services, we provide comprehensive, professional and person centred Support Coordination services to NDIS participants across New South Wales (NSW), Victoria (VIC), Queensland (QLD) and South Australia (SA). Our experienced and qualified Support Coordinators work alongside participants, their families and carers to help them understand, implement and get the most out of their NDIS plans — ensuring every dollar is used effectively and every goal is pursued with purpose.

What is Support Coordination?

Support Coordination is a capacity building support funded under your NDIS plan. It is designed to help you understand your plan, connect with the right service providers, and build your confidence and skills to manage your own supports over time. At Hope Health & Care Services, we go beyond simply connecting you with providers — we walk beside you every step of the way, advocating for your rights, resolving service challenges and ensuring your supports are always working in your best interest.

Our Three Levels of Support Coordination

Our In-Home Support Service is suitable for NDIS participants who:

Level 1 – Support Connection

Support Connection helps participants build their ability to connect with informal, community and funded supports. Our team assists you in understanding your NDIS plan, identifying appropriate service providers, and establishing the connections you need to begin your NDIS journey with confidence. This level of support is ideal for participants who need some initial guidance but have a strong informal network around them.
Level 2 – Support Coordination

This is the most common level of Support Coordination funded under NDIS plans. Our Level 2 Support Coordinators work closely with participants to:

● Understand and implement their NDIS plan in full
● Source, negotiate and coordinate supports across multiple service providers
● Resolve points of crisis and identify long term solutions
● Build the participant's capacity to self direct their supports over time
● Prepare for and participate in NDIS plan reviews
● Monitor the ongoing effectiveness of supports and report to the NDIA as required

Our coordinators are experienced in working with participants across a wide range of disabilities, including physical, intellectual, psychosocial, neurological and sensory disabilities, as well as complex and high needs cases.
Level 3 – Specialist Support Coordination (SSRC)

Specialist Support Coordination is a higher level of support designed for participants who face more complex barriers and require a specialist coordinator with advanced skills and experience. Our Specialist Support Coordinators are highly qualified professionals who work with participants experiencing:

● Complex support needs involving multiple services and systems
● Psychosocial disability and mental health challenges
● Out of Home Care (OOHC) and child protection related circumstances
● Crisis situations requiring urgent intervention and coordination
● Transitions between hospital, justice, housing or other systems
● Situations where informal supports have broken down

Our Specialist Support Coordinators work closely with allied health professionals, government agencies, legal teams and community organisations to ensure a consistent, safe and holistic support environment for every participant.
