- Continence, catheter, stoma and PEG feeding support — We provide catheter care and monitoring, stoma care, bladder and bowel assessments, continence management planning, and hygiene education for participants and carers.
- Chronic condition management — including diabetes care, respiratory care, regular monitoring of vitals (blood pressure, glucose, etc.), and ongoing health reviews.
- Post-hospital / post-surgery recovery care — supporting you during recovery periods at home, including follow-up care and nursing oversight.
- Health assessments, care planning & clinical reviews — comprehensive health assessments and personalised care plans tailored to your needs and NDIS plan.
- Nurse-led health education & support for carers/families — training, guidance, and support to help participants and their support networks manage health conditions with confidence.
