At Hope Health & Care Services (HH&CS), we offer tailored, person-centred in-home support to NDIS participants across NSW, QLD and VIC. Our goal is to make daily life easier, safer, and more fulfilling, helping you stay in the comfort of your home while maintaining independence, dignity and connection to community.

What Is In-Home Support?

In-Home Support provides practical, compassionate assistance with everyday tasks and routines. It is ideal for individuals who may face challenges managing certain aspects of daily living due to disability, health conditions, or reduced mobility. Rather than focusing on medical care, our support emphasises independence, quality of life and personal comfort.

Our support workers are trained, vetted, and committed to respectful, inclusive care — giving you the help you need while honouring your preferences, schedule and goals.

Who Can Benefit

Our In-Home Support Service is suitable for NDIS participants who:

  • Have a disability that limits their ability to manage daily personal care or household tasks.
  • Need occasional or ongoing support to maintain hygiene, mobility, meals or home cleanliness.
  • Want to stay living at home but need help to remain safe and independent.
  • Are self-managed, plan-managed, or NDIA-managed under NDIS.
  • Prefer support that is personalised, respectful, and adapted to their pace and preferences.

We welcome participants of all ages, abilities and backgrounds, including those with physical, cognitive, sensory or psychosocial disability.

Our In-Home Support Services Include

  • Personal Care & Hygiene — Help with showering, bathing, dressing, grooming, toileting and general personal hygiene.
  • Meal Preparation & Eating Assistance — Cooking meals, meal planning, safe feeding support, peg- or tube-feeding, and mealtime assistance.
  • Mobility Support — Help with transfers, use of mobility aids, getting in/out of bed, safe movement around the home.
  • Domestic & Household Help — Light cleaning, vacuuming, dusting, laundry, changing bedding, dishes and general household chores.
  • Home Management, Organising & Safe Living — Maintaining a clean, safe and accessible living space, managing clutter, rubbish removal, and ensuring a hygienic environment.
  • Life Skills & Daily Living Support — Assistance with budgeting, meal planning, household routines, personal organisation, and day-to-day tasks that enhance independence.
  • Community & Social Support — Help with planning outings, community access, transport to appointments or errands, support to maintain social connection and participation.
  • Flexible Support Hours — From short visits (a few hours) to regular ongoing support, day or night, depending on your needs and plan.
